As part of its commitment to addressing residents’ health concerns, the Iredell County Health Department is collecting supplemental data on cancer diagnoses.
North Carolina law mandates cancer reporting by healthcare facilities. To enhance compliance, the Health Department is asking residents who have been diagnosed with cancer in the last decade to complete a brief survey.

While not a replacement, this survey aims to provide timely and comprehensive data to the North Carolina Central Cancer Registry. Upon receiving completed surveys, the Iredell County Health Department will forward them to the NC Central Cancer Registry. The registry will cross-reference with internal records to ensure proper provider reporting.
The Iredell County Health Department is committed to addressing health concerns affecting county residents. Collaborating with federal, state, local partners, and elected officials for over six years, local health officials have focused on heightened thyroid cancer rates in two Mooresville ZIP codes.
The Iredell County Health Department is committed to protecting confidential health information. At this time, the department is unable to accept electronic submissions of the cancer surveys.
Surveys can be mailed, faxed, or dropped off at any of the Iredell County Health Department locations:
• Statesville: 318 Turnersburg Highway, Statesville NC 28677
• Environmental Health Offices at Building Standards: 349 N. Center Street Statesville, NC 28677
• Government Center South: 610 East Center Avenue Mooresville, NC 28115
• Surveys can be faxed to 704-878-5357.
• Please make sure all completed surveys are addressed to the Health Director.
